Major Issues: Many trials are too small to reach reliable conclusions or duplicate other research.
The authors call for more reliance on “adaptive clinical trials”—which can be adjusted mid-trial—especially in low- and middle-income countries.
One issue in LMICs is maintaining research infrastructure for such trials—but now is the time to make these investments, the authors argue.
“We should have had all this ready to go years ago," says Edward Mills of Canada’s McMaster University.
